2 > REGISTRATION FORM (table)

If you have a large number of agents to register, you can fill in the table.

Essential documents to trigger insurance in case of accident. Submit the current season's "Sports Medical Examination", along with the agents' data on registration and subsequent renewals. To report an accident, fill in the "Claim Report".

  • What to do in case of an accident?

    The claim must be sent toseguros@fpcapoeira.org within a maximum period of 48 hours after the occurrence of the accident.

  • clinical assistance

    If the Claimed Person needs medical assistance, the following rules must be observed:

  • Franchise and IBAN

    You must prepare the payment of the excess amount of € 100 for the IBAN

  • Expenses refund

    Expenses sent to the Insurer for reimbursement to the injured party will be paid by bank transfer, so it is necessary to send proof of the respective IBAN, which also contains the identification of the account holder (ATM vouchers are not valid).
